sorry to hear the problems you are experiencing with Bluetooth and the headphone volume in-call. my suggestion is to perform a soft reset (pressing the volume down and power buttons together until the phone vibrates). also check that you have all the latest Nokia system apps installed from the Store. if this doesn't work, then check if you have apps such as Skype installed as this can cause conflict with the OS, and i would recommend uninstalling it and trying again.
if those don't work, then you need to perform a hard reset (Settings > About > Reset your phone). please bear in mind that this wipes all personal data, so please backupe everything to PC and / or online storage first. failing that, you will need to take your phone back to your point of purchase i.e. T-Mobile, and if they are being less than helpful, to your local Nokia Care: http://nokia.com/support
